Good afternoon.
Are you with friends on your LIVE friends list when this occurs? If so you might have NAT compatibility issues with one of your LIVE friends. I know I have this particular problem with a certain LIVE friend on Halo: Reach and Halo 4. On Halo 4, I randomly am disconnected from the LIVE party and LIVE and when I reconnect and try to rejoin, I get the “You cannot be added to the party queue.” message, forcing me to reset my console when with that one LIVE friend. When I am with that one LIVE friend on Reach, my armor seems to reset to all recruit and my service tag disappears, but when he leaves the party, all my stuff comes back to normal in game.
Another potential issue can be disc damage and/or file corruption (if you have the downloadable version). Check your disc for damage and clear your system cache. If you disc has a lot of scratches or a big scratch, you may want to buy a new physical copy of Halo 4.
Lastly, your NAT might not be open. Not having an open NAT can cause what you described to happen. There is a set of instructions you can google if this is the case to change your NAT to open. Oddly enough, playing Halo: Reach puts my NAT to open if it is moderate, but only if it were that easy for the other 99.9%.
