Interruption of the connection to the host. I think.
The coolest incident of this was for me was in Snipers on Damnation. Terrible host - super laggy. All 4 of us respawned in the same room (the one connected to bottom snipe via the tunnel). None of us could move. Then, there was the sound of a single sniper shot, and we all died simultaneously.
