As I was building my ship in Forge, I noticed that a few of the [FX: FIRE HUMAN; THRUSTER; LARGE] no longer appears visually but the blue flame it emits does. The same can be said for [FX: HOLOGRAM COVENANT; GALAXY; MEDIUM]. A blue light appears where the center is supposed to be but there is no spiral galaxy revolving around it.
My FX percentage on the map is 90%. Could this be why?
If you are viewing in Forge and the FX are not appearing then the amount of FX is what is causing the issues. Try to limit the number of FX items (e.g. teleporters eat into this budget, so replace them with invisible ones if you can). Alternatively, you could script them so they only appear when players are in the area.
If it looks fine in Forge but they are simply not appearing in custom games, then this is a common bug when there are a large number of objects in general. Try to limit the number of objects to 1300 and then items will begin appearing as they should. Additionally, as before reducing the FX can improve results, as well as decreasing the lighting.
This is a very annoying bug with forge, but hopefully the work arounds above will improve things.

The FX are not appearing in Forge or in Custom games. Which is odd because ones similar to them are (for example, I have around 24 large human thrusters and all appear correctly except from 3 and it will always be 3 but not the same 3). They showed in one game and then they suddenly stopped showing.
It’s really quite annoying too as I plan on using the ship in a machinima.
Exact same happened on my map “Escape The Jungle”. I’ve got a large waterfall that used 60 steam pieces (and some bubbles). FX was at 110% and randomly (again not always the same emitters) wouldn’t show. I had to reduce it to 30 steam pieces and the majority show up now.
I’ve got another map that uses fire, but it sometimes doesn’t show (but confused players still take damage from the area).
So, it seems it’s definitely a common bug, and there’s nothing that you are doing wrong that is causing it.
You’ll just have to cut back where ever possible.

One more question, if I may: what contributes to the FX percentage? Because I only have those 24 thrusters and the galaxy spiral light. Surely they wouldn’t take the percentage all the way to 90?
Anything labelled under FX will work it’s way into the budget, as well as anything that has animation, such as the blue light at the base of the UNSC teleporters. I believe the FX of the map (in map settings, e.g. fog) also contributes to it.
The size of the FX object also affects the limit. Large FX pieces like large fire and lightning (and possibly those thrusters depending on their size) will contribute more.
Ah, I see. I may have to rethink my ships engines then.
