I understand it a little better now, right, you are correct bans are not monitored and probably never will be, it simply takes WAY to much time to monitor all bans and then to have an appeal process, it would be to much work for one man or a team to do, considering investigations accounts would have to go under similar to how Xbox LIVE accounts do when stolen.

As for the monitoring over three games, I actually misread this a while back in the original post and only noticed it recently:

It doesn’t say it takes three games (like TS on Asylum, FFA Slayer on Powerhouse, CTF on Hemorrhage), it means it has been updated since Halo 2 to improve the accuracy of ban hammer, so technically, it can only take ONE game to be banned, not two, three or more, just one.
Onto quitting and getting banned, if you are simply quitting, you only quit bans which are TEN minutes long, you will NEVER get credit banned for quitting games on Halo: Reach.
As for the account & console bans, account bans are given to players to stop them from boosting/cheating on said account, the console ban is then handed out to stop said player from going onto a second account to play on another account to still earn credits, making the original ban pointless.
