Played ‘The Ark’ on Legendary for 3 hours and died 58 times, so painful. I never entered a vehicle but the achievement didn’t unlock. Reseted TMCC and also reseted the Xbox, but still nothing. Is the achievement bugged?

It could be. Did you save and quit at any point in the game?
No, I did not. Played the 3 hours straight.

Did you have any skulls on at all? Sometimes putting skulls on it will void the achievement somehow.
I think I had the Grunt Birthday Party skull on.

Play the level with no skulls on whatsoever.
