It’s a known issue caused by the latest update. It’ll be fixed soon (hopefully).

Whaaat? Not once did I reach a Campaign level called “Me Out Of The Game”. Must of played the game wrong lol!

Short term fix has been suggested to just restart the mission you’re on from the beginning. Obviously if you are near the end of a legendary level, this may be a terrible suggestion, but so far it is all we have.

We’ve been looking into issues surrounding Saved Games and Checkpoints in Campaign that can end a player’s session and bring them to the Xbox One Dashboard. This issue can be resolved by restarting the mission from the beginning.
