By far, Cortana’s death in Halo 4. Her goodbye scene at the end where she is finally able to touch the Chief had me in tears.

Obviously Cortana dying. The song “Green and Blue” is just to sad by itself and then comes the seen. If you can resist crying, you do not have a soul.

You could tell how much he was hurt just how he could not look at her in the eyes. His best friends were Johnson (KIA), Cotana (now KIA), and Lasky (only recently). Who will be AI for Halo 5? Will Cortana come back? I do not know, but if it stays where it is, it will stay on the number one spot of saddest video game moment ever, or at least, for me.
