I like to turn up the brightness, and turn on subtitles whenever I play Halo 4. Every time I turn the game on, the brightness is back to the default of 3, and the subtitles are back to the default of ‘automatic’. Is there a way to make the settings permanent, or is this a glitch?

It seems to be consistent with all games from Halo 3 on. If you have a hard drive, you can retain the settings as long as you don’t play another game. However, playing another game and then playing Halo 4 again will cause the settings to reset. There is no way that I know of to retain this setting, but there may be a way that I am unaware of.
One other problem is that a lack of a hard drive usually causes the settings to reset after you leave Halo 4, even if you return to it immediately. Sorry about that.
