Apparently sometimes this is being caused by individual missions if you press exit instead of continue on the mission completion summary/score screen. If you know which missions this happened with go back, replay, press continue and see if that fixes it.
Other theories regard if you load a save game but there’s inconsistent information there.

Skulls my waypoint record and game mission menus say 15/15, but cheevo says 93% (14/15), Bonus objectives the game mission menus say I have them all, waypoint says 64/65, and the cheevo says 97% (63/65). All mine are Solo XBOne (though have also played Coop on PC) am UK

Xbox and UK but its shouldn’t matter if I was in the congo playing it on a hippo (although that would be impressive). This is more important than nerfing locusts as this isn’t something you can work around

Totally agree with you about the importance: however for fixing the bugs it is relevant info: they ask for it in the sticky at the top, plus for HW2 the XBox/PC difference for this is important, and location gives hints: e.g. the European servers are less stable so you get more disconnects so this might be more common for European players (and proportionally I’m seeing a lot more European complaints about this one than US so wondering if that’s useful info).

Crap, I did that on a few of them and I can’t remember which. It’s stupid though that I have to go back and replay missions because the achievements are screwed up. I don’t understand why 343 can’t release a game without problems. Halo 4 & 5 didn’t have achievement problems, but Halo 4 didn’t have a very good forge or multiplayer and Halo 5 had an awful story and was incomplete on release.
