In Halo 5: Guardians, there’s an achievement titled “Off to the Races” which requires players to complete 5 matchmade games of Breakout. However, with Breakout seemingly being permanently removed from the playlists, this achievement is left in an unobtainable state. Even with achievements only existing for bragging rights, removing the requirements for an achievement should absolutely never happen in any video game.

Uhm…Elimination in ranked is all just breakout modes. So just play that till you get 5 match-made games done and there ya go.
I believe breakout is now under elimination in arena.
Breakout is in the Elimination playlist. That playlist has 3 modes, Breakout, Breakout 2.0 and Extermination(which is a variant of Breakout only in some way) so play 5 matches(or more if Extermination doesn’t count towards the achievement) in that playlist and you’ll get it.
Also, Action Sack playlist also has game modes having some fan made variants of Breakout. So those should count too probably?
Extermination games themselves also count as Breakout, right?
Anyway, you’re right. Achievements should never become unachievable. 343 needs to fix “A Storage Solution” in Halo Reach.

That achievement actually is still achievable, it’s just you gotta get lucky with it, I’d say try and find the smallest file size for a file so you’ll have much better luck with it.
