The description says:
“Complete a War Games match with at least 15 kills without dying”
I just went 16-0 on Capture the Flag, which is quote “a War Games match” and it was never awarded. Boo!
I had something similar happen, because apparently halfway through the game the servers crapped out for me.
this is like the Perfection commendation, i think as far as a few people have said the Perfection commendation can only be earned in Infinity and Big Team Slayer matches
it is possibly the same with the daily challenge, its happened to me before playing CTF and i never got it towards my commendation
so try it in one of them other playlists
The sole definition of the Perfectionist for bungie was- Going 15-0 in any slayer mode. They really dont high light it in Halo 4, but im going to assume it’s the same here.
I just had the same thing happen, but I did it in Infinity Slayer. 16-0 and my Commendation Perfection - Novice gave me XP after the game but I didn’t get the challenge.

Maybe the Challenge is supose to be activated via medal that doesnt exist any more.

No, connection was fine the whole game. I was also there from the very Begining of the game too. I got the First Strike medal as well. So I definitely completed the match.
The challenge isn’t properly worded it should achieve a perfection in wargames matchmaking. You can only get a perfection in a slayer gametype (maybe regicide also im not sure)

Yeah, I got all the fancy medals.

Im guessing that the coding around this is just defunct. I Certainly dont know what to say about this cafuffle.

I was referring to the OP in your case I’m guessing you must have switched host mid game.
Just another example of 343i’s laziness…
