Here’s the video on where they are at, i think you may have been searching in the wrong places
They don’t show up if you are playing in classic graphics (the old 2001 graphics).
Worst case you have a buggy game disk 
Sign out and restart the Silent Cartographer again (heroic this time).
Difficulty doesn’t matter. Are you sure you’re playing with the new graphics? Skulls don’t appear in classic mode.

Hey, that is not stupid. Getting skulls in H2 was a challenge in itself and it encouraged exploration. With H3, I just wanted to get it over with. Same thing with Anniversary. But with H2, I took my time. I liked how the effects were immediate so I didn’t have to restart the mission to see it. The scenery (i.e. berserking Elites) was a nice touch too, giving me a reason to stick around after retrieving the skull.
