Has anyone encountered a crash or a “fatal error” while playing matchmaking and it result in a temporary ban? I just had it happen to me twice for the first time since playing the game upon release. Do multiple temporary bans add up to more serious punishments? Any why hand out temporary bans upon a crash or fatal error anyway, isn’t a game randomly crashing a well known thing that happens from time to time on PC?
The game doesn’t know why you left a game, it just knows that you did. The game cannot detect if you were “right” in leaving, such as in the case of an emergency or the game crashing. It’s happened to me a few times, just shrug it off.
I get that is what the case is, but most modern games don’t penalize a crash and actually offer you the ability to rejoin a match or your team in progress. Games like Gears 5, Warframe and Destiny 2 do that, I am sure many others to it too, so it would be nice if 343i implemented something like that rather than slapping the players with the punishment. Not even a error report is generated.

Exactly. You should be able to rejoin, and if you do remove the quit penalty.
Its a simple solution for a game full of crashing/disconnecting issues
