Switched On achievement did not unlock (XB1)

So I completed Operation B today and I got the Ballistic Backfire challenge but the Switched On achievement for completing the operation didn’t pop.
When I go to my achievements the progress bar fills to 100%, but the achievement remains locked. It is also locked on the in-game achievement page.

Has anyone else had this issue? And if so, were you able to fix it?
