My friend’s account showed up as suspended for creating an alt account. He didn’t and has only ever logged in with the one account. He hasn’t been banned for anything else either. He can’t contact anyone directly to ask them to reconsider. Does anyone have any advice? It seems like a lifelong ban for a mistake he’s pretty sure he didn’t make is a bit much and it seems like there should at least be a way to plead his case.

Inferring this pertains to a forum ban, your friend should send a private message on their account to one of the moderators on this list, explaining the issue. Banned users can still send private messages.
