For Reach, are you playing on Legendary?

For CE, check your campaign stats on Waypoint and complete the levels which do not show up as completed. Achievements are tied to Waypoint.

I know there is an issue with ODST audio logs. Google the problem and you should come across some tips on how to fix it.
