I–and many others, if Google is to be believed–am having a constant issue with being kicked back to the main menu when I attempt to start a chapter of Spartan Ops. I am playing solo. Disc One won’t install to the HDD but Disc Two and Season Two are installed. Sometimes, I can play a chapter but won’t be able to progress, afterward.
Have u tried redownloading the spartan ops thing? (btw its season 1 not 2).
Or try reinstalling the disc 2 as well.

Just to make sure you have everything covered, I would first suggest backing up your save files and such either to the Cloud or a different Device. Then, start by clearing the HDD’s Cache (go to System Settings, Storage, highlight your HDD, press Y and choose “Clear System Cache” I believe.) Finally, go into your HDD, highlight Halo 4, Press Y and Delete. Hopefully this will clear out any hidden data issues, should there be any.
Also, you say that Disc 1 won’t install to the HDD- can you provide more details? I’m assuming that you obviously have plenty of disk space, so for example: when you go to Game Details for Disc 1 as if you were going to install it, is the Install option there? If so, how is it preventing you from installing? Stops halfway? Doesn’t start at all? Can you install other games? If so, what happens if you delete a game you already have installed and immediately try to reinstall it?
If the option is there but it doesn’t Install, then it could be an issue with the HDD itself, while on the other hand, if the Install option is NOT there, then for some reason your Xbox thinks it’s already installed.
In any case, this is where I would start:

Turn off the Xbox, disconnect your HDD, connect a different storage device (I don’t think storage capacity matters, but if I’m not mistaken, Disc 1 is just under 5GB, so 8GB and up should be fine for this test,) and then go back into Game Details to see if the option has appeared. If so…

Backup your save files and format your HDD. Other than that, you could contact Microsoft to replace your HDD, or buy a new one.

P.S. - You could also try loading up Spartans Ops from Disc 1 with no storage devices as well.

What kind of Xbox do you have?
FYI: A 4gig is not an HDD.

Will you please send us the following information via private message for investigation:
- Console ID (you can find your Xbox 360 Console ID using these instructions: http://connect.microsoft.com/XboxLIVEUpdate/content/content.aspx?ContentID=14068
- Service Date (found on the back of the console).
- Serial Number (found in the System Info screen or on the back of the console)
- Product ID (found on the back of the console)
I completely forgot to update this post. I have forwarded my information to 343. I assume they are still trying to figure it out. But to answer the questions raised in this thread, I have the 250GB HDD with over 190GB free space. I have cleared the cache numerous times and, when I was initially having issues with Campaign, cleared all the game data.
Now, the issue has changed slightly. I still cannot usually start a chapter of Spartan Ops but, the 1 out of 300 times I can start, I can play through to the end, then when the UNSC logo comes up after completion, it freezes and I get an error about the disc being unreadable. Other discs are not having this problem and all other console functions work properly.
It should be noted that the disc has a strange blotchy area on the encoded surface, sort of like a watermark that goes all the way around the disc, covering between 1/3 and 2/3 of the silver surface from the center. I believe the disc is bad but can’t return it and I’ll be -Yoink!- if I’m going to pay Microsoft 20 bucks for the “privilege” of having them replace a faulty product. I may rent a copy from Redbox and install it, then see if that fixes the issues.
I have taken a picture of the disc so you all can see what I’m talking about. I inverted the color to make it easier to see but it is easy to see with the naked eye if you just hold the non-printed side of the disc up and tilt it back and forth in the light.
use a microfiber cloth and clean it and see if that fixes the problem and if not add a few drops of purified water to a corner of the cloth and clean it with that
I tried every cleaning method known to man before going any deeper on the issue. The first thing I always try is to make sure the disc is clean and has no scratches.
In any event, another update: I now have a 250GB 360 Slim. Tried to install the disc and, again, got a failure around 40% with a “disc cannot be read” error. It’s become pretty clear to me that there were an unknown number of bad disc pressings made and shipped. It’s also, sadly, become all too clear Microsoft and 343i intend to do absolutely nothing to make this right for those of us who got the bum copies.
I am having the EXACT same problem…I tried doing everything.I cleared my system cache, re-installed Disc 2 and recovered my gamertag. Nothing happened.Even I had got the disc unreadable error even though my disc was completely new.But after 3 tries, it eventually installed successfully.
Another update. It’s still not installing Disc One to the hard drive of the 360 Slim but it is no longer giving me problems running Spartan Ops. Probably two unrelated issues. It’s possible the slower internet of my old 360 (or imminent failure) caused the Spartan Ops issues, which would explain why it works now. Still looks like a bad pressing of Disc One but shouldn’t be too big a deal for online play.
