I didn’t receive the code by email, instead I got an Xbox Live message saying I hadn’t used mine yet, giving me mine. But when I entered it, it said it was already used.

Your not the first person to have this problem nor probably the last. Best advice I can give you is to call up Microsoft tech support and give them the run down, I imagine they’ll try to help you out or direct you to someone who will.
same exact thing happened to me. Xbox support said my best bet is to post something on the 343 forums. I am doing so here at waypoint and at the 343 website. I have the sinking feeling that my efforts are futile.
