I completed all Spartan ops missions on legendary (solo) and the achievements smooth operator, Icarus, and hazard pay are all stuck at 96%. Which means the game didn’t count 2 of the missions for some reason. Is there someone I can get in touch with that can fix this?
Exact same thing happened to me, except I’m stuck at 98%.

One possible fix… remove your profile from your xbox and log in again. Forces a redownload of your data which can force sync it, then relaunch the game and try another mission

I had a slightly different problem yesterday in which I did not have all of Episode 5 done for some reason and the achievement were glitched I thought, when in game I did all of the episode on Legendary already and I was stuck on several difficulty achievements with an odd percentage (which it should be an even percentage actually). So I was stuck at 80%, not knowing which chapter was causing the achievements to not had its progress completely properly. I fixed this by replaying 4 of the 5 chapters and for some reason, chapter 3 was somehow incomplete for me when the first time round, I already completed it on solo on legendary and once I completed that chapter, it game me the updated progress with the other difficulty achievements.
Bottom line is you might have to replay certain chapters to get the achievements to properly unlock. Do you have any episode achievements left incomplete or are they all completed?

I just finished Spartan Ops on Heroic but the Hazard Pay didn’t pop up. It says I have 98% of it done but I have the heroic logo on every mission under solo. After I finished the final mission and the achievement didn’t pop up, I looked on the achievement screen and there was a small white bar, indicating I made progress. Should I replay the last episode? Is there a way to fix this without deleting and redownloading my profile?
I have the same problem, finished all in legendary and achievements haven’t popped up yet its been more than 3 months. Help :’(
This is bugging me because I have the same issue saying I’ve technically missed two as it is sitting at 96% on tracker
i have replayed several episodes and it still hasn’t gone up. Clearly states all are Complete on legendary
I’m stuck at 98%.
Had this same problem completed s[prtan ops a year ago and the achievements didn’t unlock just started replaying the game and found were it said only completed 48 of 50 chapters so replayed them from the start i found episode 8 chapters 4 and 5 were the ones that glitched the first time so hopefully this is a place to start if not then just play all the levels again
I’ve played through all 50 missions on legendary solo TWICE now and still stuck at 98%
Almost 2022 and this still hasn’t been fixed.
