Semi-Automatic guns stop firing while in gunfights

I started playing the rest of the MCC recently (games aside Reach.) and noticed that when using semi-automatic weapons (like BR, DMR, Carbine, Magnums, etc.) in the middle of gunfights, in one of the several times that I pull RT to shoot, the weapon doesn’t fire. It is very enraging when it happens, specially when I’m winning the gunfight. Is anybody else experiencing this?

IMPORTANT NOTES:
-I play on PC (Steam) and use a 1st gen Xbox One controller plugged in with a USB charger cable.
-I already have verified the integrity of game files on steam.
-I have tested my controller multiple times on the Windows control panel’s controller calibration settings and I have not seen any failures. (I have spammed buttons and etc.) So I highly doubt it’s my controller.
-I have experienced this usually only while in the middle of a player vs player gunfight. I haven’t experienced this in any offline game modes yet.
-The games I have experienced this in are Halo CE, 2/A, 3, and 4. (I haven’t experienced it on Reach and ODST yet but it is likely to happen there as well.)
-This issue shouldn’t been confused with hit detection issues.

Also just for fun, I want to simplify everything just in case: The "RT"s down below represent when I pull RT to shoot, and the "X"s represent the shots.
