A lot of people where speculating that Requiem is going to be destroyed by chief at the end of the campaign. But it’s not because Spartan ops takes place 6 months after campaign.
And they said you will encounter Chief in Spartan ops that are cool. You probably encounter him on the deck of UNSC Infinity because it looks like chief gets aboard it pretty early in campaign.
But that still doesn’t explain the mysterious main menu…
For some reason I was hoping it would be the Infinity, but that won’t happen.
And Requiem doesn’t have to be completely destroyed. That screenshot shows a kind od big hole in it. Maybe they busted out, blowing a big peice out of it.
And Spartan Ops takes place 6 months after it, they said some environments will look familiar, but changed still. Maybe its the environment reacting badly to the damage.
Cortana may be made into a different AI, but I don’t think it’ll be in Halo 4.
Kinda reminds me of what happened to the ring in Primordium. Maybe the ring and planet are a close next door neighbor of requiem and will take a huge roll at the end of halo 4 or maybe a sign of halo 5. Highly doubt it though.

it could possibly be some spartan ops are on their which is partially blown up while the others are on the ring in the background
