1: What was going on with red and blue team while church was in his world?
2: Why is Carolina alive at the end? I thought she was dead; back in season 6, delta said that she was killed by having 2 AIs in her head.

As far as I was aware Delta said;
“She only functioned for a short time, and not very well while she did.”
I don’t remember anyone saying she was dead.
Well I assumed she was dead…
Ok, let me rephrase the question: Why in the hell is being evil?!
And another thing; where the hell is Washington?

How is she being evil? She wants to get back at someone who screwed up her life and the lives of many other people. As for where Wash is, depends which Wash you’re talking about. Flashback CGI Wash is probably somewhere is the base, Blue Team, Halo 3 Wash is possibly with the other Reds and Blues, we heard Grif say “we’re taking fire” so clearly a group of them are fighting.
… damn it all. Season 10 better be good, otherwise, I’m gonna pissed.
As for Carolina, I expect great things from her.
