Progress Reset (Campaign wise and terminals)

So I decided to unlock the Raider torso through waypoint by entering in the code. It wouldn’t let me, even though I beat the campaign on Legendary solo.
So I decided to check my story progress and it said I never beat the campaign, even though I got all the achievements in campaign and beat it on legendary solo day 1 and still have the Mark VI armor unlocked and the visor. On top of that, the story terminals have been reset.

Anyone have any advice on this or have any solution to this problem, other than going through the campaign again?

i had a problem similar to this i was on mission 8 when i caught it. it said i never completed mission 2 , even though i did complete it. so i had to go back to play mission 2 and eat my progress in mission 8 had to play it over to get everything
