My brother and I both just completed the level swords of sanghelios on heroic co-op together and both our mantis survived but the achievement didn’t unlock for either of us.
I had this happen to me as well, and we got the achievement by walking the mantises up to Arby at the end of the mission. Apparently one or both of the mantises have to be at the top of the stairs. This worked for me, hopefully it works for you too.

i ended up going back and doing that and it worked. very weird that it’s programmed that specifically.
