Well, we already know Halo 4 will feature the UNSC (the UNSC Infinity from the Concept Art), but it’s possible that Chief never makes it back to Earth and remain MIA in offical records.

Heroes Never Die was talking about the legacy of heroes, and how their fame keeps them from being forgotten and being truly dead.
Spartans Never Die was an actual decree where spartans are never declared KIA to increase morale.
I dont see the connection between these and your comment. He will be know as MIA because he is literally Missing in Action, one of the few Spartans who really was.
