This is happening for the SINGLE player as well. Meaning I literally can’t play the game that I went out and got at midnight and took off today to play.
i am having the same issue. I played campaign just fine last night, and got to the level infinity rally point charlie. I try to play it this morning and it won’t let me load the map, it gets to about 40% and I get ‘players failed to load content’. I know the online servers are having problems but this sucks I can’t even play campaign. I tried loading rally point alpha, bravo, and charlie. No luck.
So I just played a ton of the single player (well, not a ton but probably a level at least) where it would have had to load from the disc.
Now I’m installing the single player disc to the hard drive to make sure this never is an issue again. Strangely… this failed last night but I decided to ignore it.
I’m guessing that the game is unforgiving about load errors, that the discs were made at a high rate and that the 360 has had a variety of different dvd drives probably some of which are crummy.
tl;dr: keep giving it a shot, try installing the SP disc to your HD so that once it’s done, you never have to worry again.
I can’t wait until the Xbox 720 does away with optical media. So silly.
I’ve been trying, but it won’t install. Installed perfectly on my friends, so it’s not the disk. And no, my Xbox isn’t broken, I have no trouble installing other disks.
I use my 250g HD, I have also tried using cloud save.
4.I hit resume campaign, and the error appears. If I manually try to select the mission, same thing. The only mission I can start is the prologue, and it’s laggier than usual.
I played campaign 100% fine last night, and had no problems whatsoever. How could server issues etc affect even offline single player play? I highly doubt that this is a server problem, but more-so coding/disk errors.
EDIT: My game just loaded a mission. Huzzah! And my profile synced. I guess posting on the forums solves all of your problems. I’ll leave the previously noted, for reference in-case anyone else is struggling.
All attempts to install the disc bring up an “Disc is unreadable and may be scratched” error. Yeah, it’s not scratched. Bought it four hours ago.
Using my 60 gig hard drive, still has 27 gigs left.
I haven’t been able to get past the opening prologue video yet. After the interrogation scene, it brings me back to the main screen menu. Attempts to load first mission will begin loading to 30-60 percent and then stop and say “Players failed to load content”.
Multiplayer works fine. Played a few games, tried the campaign again and its the same deal. Is this a problem with my own xbox, or is it the disc, or is it a server problem? I just need to know if I should take it back and get another one, or what.
I had no problems with installing the game in my HD.
My HD of 20G, 8G left.

20GB Xbox 360 Normal
It is installed.
I used both and it seems to make no difference. Currently using hard drive.
Its various, sometimes I can get past a part I couldn’t earlier and sometimes it takes a bunch of tries to get past. I can also play multiplayer and spartan ops fine.
It’s still not working. I was able to play it just fine on campaign until I decided to do the update. Once I was updated then all of these problems started happening. Can’t play campaign or multiplayer.
Well at least I’m not the only one that is having problems trying to play the campaign (same situation as everyone else can’t play the first mission after the prologue cutscene), luckily everything else works fine.
1. What type of console are you seeing the problem on? Original Xbox 360 with chrome disk tray.
2. Did you install the game to your hard drive? No my Xbox won’t allow, says it scratched when it is brand new. Although I will keep trying.
3. What storage device are you using for your saves and profile? 120GB HDD (22gb left)
4. When the load error happens, is it for all levels and checkpoints? Can’t answer that as I’ve not been able to progress in the campaign.

I tried, but it fails at 25% every time and says the disc is scratched/unreadable.

Started off with my profile on MU and played campaign without Xbox Live. Then felt like I should get the update and once it updated, it no longer let me play campaign OR multiplayer. I deleted my profile then redownloaded it to my Hard Drive this time and it is still giving me load error.
When the load error happens, is it for all levels and checkpoints?
This happens on ANY level, whether it be campaign or multiplayer.
Anytime I load campaign it says
“Players failed to load content. A player may be missing downloadable content.”
(Be aware that I am in a party by myself and that I have installed disc 2’s content multiple times)
I’ve been troubleshooting this all day with and without network, signed onto a local account and was still not loading. I’ve done pretty much all I can think of. If there’s anything you can think of that would help, that would be awesome.
EDIT: I have also deleted my entire Halo 4 location under “Games & Apps.” Doesn’t seem to be a temporary fix in my case.
From what I’ve played, I know this is a great game. I just don’t know what’s going on here, I’m thinking it’s either hardware or disc related. I’m probably going to go exchange the game for a new one if they’ll let me.
