Are you sure it was slayer? Also, I know in H3 you couldn’t have 1 betrayal, did that happen by any chance? (not sure if thats a thing in H5)
Edit: You also have to win. I see you have a lot of SF in your game history, maybe you can’t get one in SF, not sure tho. Just a guess
Do you have a link to that game OP? I went through your last 50 games, but I don’t see any that would have granted your a perfection

What he said! It took me a while to remember that as well. I’ve had objective games where I had 15+ kills without dying and didn’t get a Perfection medal after winning. Took me a bit to remember that it’s Slayer only.
