So do you have to ONLY have the debuff skulls on to earn this achievement? I have earned a gold medal with all 15 skulls on twice now and that hasn’t worked. If it only works with just the debuff ones on, what is the best mission/difficulty to complete this?

JUST the debuff ones, do the first mission.

Rush through as fast as possible.
