I have completed the campaign today and collected the achievements for completing each mission and the campaign, but my service record r.ains completely blank, bottom level and every single mission is locked except the first and no gameplay is saved at all.
I am having the same problem.
How do I get all my saved games back?

You can’t. This problem has been going on for a LONG time now and thus far, it seems like 343 & Creative Assembly have chosen to ignore it. I don’t want to bash them but I have no choice.
