So after some failed attempts, a friend and I completed Chapter 1 of Episode 5 on normal without dying. I’m not quite sure… But I think you have to do it on Heroic 
The achievement description is “In Ch 1, Ep 5 of Spartan Ops survive the enemy assault during the defense on Normal or harder.”

Oh, you are right, my bad, I apologize. Then, I don’t now what happened. Maybe some kind of Glitch.
Went back and did it in matchmaking and got it even though I died. So, that makes sense.
I got it on the fist attempt online, just try again.
