Do you have any odd audio set up like wireless speakers combines with wired speakers and wired headphones etc?
Can you try plugging a set of earphones directly into the Audio-out of your PC?
Most of these issues were cased by the end user having a convoluted audio configuration. If you have Realtec audio manager, check what the udio out device is set there, else check that in Windows audio manager and set one device to default, disable the rest.
