My friend and I just completed the Library on Heroic and he didn’t die at all, yet no achievement?
There were a few times at the very beginning that we did “Restart Mission” because we messed up the grenade jump. Would restarting the mission void the achievement? Otherwise, he didn’t die at all.

If you split screened it with your friend, both of you must make it through without any deaths. If you died but he didn’t, then he won’t get the achievement.
If it was over Xbox Live and your friend didn’t die but you did, then he should have received the achievement.
Also, if he reloaded the checkpoint/restarted the level/saved and quit, after he died, the game will not give the achievement as it will confirmed your friend died. The only solution for this is to go to the mission select screen and start the level progress over (just switch the difficulty and that should do the trick but make to put it back on Heroic before you start The Library again).

Yes make sure you change the level ,then back again ,don’t just restart mission .
on co-op not one of you can die not even once, it a right pain the -Yoink- but you’ll get it eventually.
