No achievement and false information ?

Hey guys,

Yesterday i completed the all the Halo campaigns on Legendary in Co-op with a friend. However, my achievement is only at 97 %.
I have looked up wich mission it could be, but i have completed every single mission, incuding intro’s and outro scene’s for all the campaigns on Legendary in either Co-op or single player mode.

I really do not understand what could be causing it, since Halo CE is on 10/10 , H2 is in 15/15, H3 is on 11/11 and Halo 4 on 10/10. Suffice it to say that i am also not getting the achievements for easy, normal, heroic and legendary ofcourse.

Someone having the same issue or know what the problem could be ?

I just beat halo 2 on heroic and it isnt giving me the achievements for normal or heroic. On missions it says i beat heroic on all 15 then i go to career and it says 12/13 doesnt make since
