Every time I get into a game after a couple seconds it will kick me out and say “Left Fireteam”… Does this happen to anyone else ??
Occasionally, maybe it’s your internet connection.
Has happened to me and I can confirm that it was not my connection. Sometimes the game freezes afterward and has to be restarted to correct.
H5 is very annoying lately, especially after the update.

Hard reset your console, make sure you aren’t set to appear off-line, and don’t have multiple accounts logged on and it should resolve itself.
