At complete random the game will say you have left your fireteam and kick me out of the game. I have now been banned twice from this, and it has only kicked me from the game twice. It was also been happening for two days now.
If you are not wired, I would try that. It is very rare that there is a disconnect with wired, and when it has happened like with the error you described, it is pure or post game, so it doesn’t count against you.
Had that happen to me a couple times, gets annoying especially when you get banned.
