The code card I do have is just a standard 14 day trial. Not a Limited Edition Fotus card one.
So I’m a little concerned that they may have packaged this copy wrong.

I’ve never bought from Amazon but if it’s anything like Ebay then you should contact the seller and tell them the situation. Other than that you may be SOL.

I have 4 Xboxes actually.
My oldest is a 2007 one also, it is a 12 gb console and the damn thing’s tray doesn’t even open, it’s been through 3 RRODs and it’s pretty much a paper weight.
The first Halo 4 xbox I got was to replace the white one downstairs in the living collecting dust. The second is my own for my room so I’m not bothered while I do commentaries and gamings.

On the card, does it mention ANYTHING about an Emblem?
The Fotus code is a part of that, along with the LR skin and the emblem itself.
If not, then yes, contact Amazon.
Just a thought, if you used a credit card you may be able to revoke the payment if Amazon doesn’t want to help.
