During the Halo 3 trailer Cortana says these lines "I know you… your past… your future . Now then did she for foreseen the coming of their survival? And did she see them going to Sigma 7?
I don’t think that’s what she meant. She maybe saying that she can tell what his actions are going to be, or what he has done. Not what will happen to them.

Im sure she didnt mean it in a literal way… and as far as the planet goes, its not a definite fact its Sigma 7

stigma 7 is not the halo 4 planet
what? isn’t the legendary planet from halo 3 and the halo 4 trailer stigma 7
