Didn’t get the medal/credit for it. Can anyone tell me if there is a way to get in touch with someone that can fix this? I want the world to know I did it even if it was pure luck!
There are no medals in Halo 4 for perfections. In addition, perfections are only awarded in Slayer gametypes so if your 15-0 was achieved in CTF or Ricochet or something, it doesn’t count as a perfection.
ETA - I went looking and I believe you are referring to this game, correct? If so, unfortunately you were two kills shy of advancing your commendation by one perfection.
That aside, you were close! You can do it again and this time get to 15. 
I am referring to that game and I uploaded the video to my file share proving that I did in fact have 15 kills. The other team quit out except one guy and caused a game reset. I think that deducted the 2 kills from me. If there isn’t a perfect game medal for halo 4 that sucks! Well I guess i don’t have a real issue then since I do have my video. Still sucks there isn’t a medal for this. Thanks for replying to me, I really appreciate it! Wait a second… you said perfect games are awarded, how are hey awarded in halo 4? Because I did have a perfect game and I want credit LOL.
Yeah no medals but if you hit start and go up to your commendations, scroll through them and you’ll see that there’s a commendation for perfections. When you get a perfection, that commendations advances. The first tier is one perfection then you start on tier 2.
Unfortunately black screens can cause issues and it’s a huge bummer at times. Sorry you missed out but you did it once, you can do it again.
Definitely check your Commendations. I had a game yesterday black screen a BUNCH, and waypoint only has me for six kills, but the Commendations did go up.
To echo snickerdoodle - you CAN do it again. I am not a great player, but I have figured out the game type and play style that works for me, and I’ve been averaging one per week - now if I could just quit falling off the map…
