I understand what the Commendation says " Complete a Campaign mission on heroics/legendary without dying" I’ve done this many a time and it has yet to give it to me i even have my Carters Helm for my Avi. to prove that I at least did it once. there a special way i need to do it?

Check your progress on the commendation and see how far off you are from the next tier.

Flawless cowboy can be done on heroic, but to get Carters helmet you need to do it on Legendary without dying.
