I’m just trying to figure this out. In the books it stated that when the bone augmentations failed, they simply deformed the bones of the Spartans, and when the muscle augmentation failed, they Spartans had uncontrollable muscle spasms. So what augmentations did kill off the Spartans?
I think it was mostly the muscle augmentations. Sometimes it caused the heart to horribly deform, other times it simply caused them to deform and pulverize their bones due to their strength.
Deformed bones and severe muscle spasms can lead to death, especially if both happen.

The stress placed on their bodies likely sent them in to cardiac arrest.
I think all that combined pretty much explain it all.
I kind of got the feeling from Soren-066s story that there was some sort of mental test that the Spartans were subjected to, unbeknownst to the scientists, that had something to do with their bodies accepting the augmentations. But thats just speculation.
The augmentations on the SIIs were pretty horrible though. Soren describes one particular aspect of the procedure feeling like glass being forced through his viens. Its possible some of the Spartans simply died to traumatic levels of pain.
Spartans never die. They only get missed in action. … means they respawn.
… somewhere … far away of their existence. So far that the word “respawn” won’t fit not longer. Because there is no time so no point in the timeline where they actually can respawn. And no location neither. Hmhh … and so on and so on…
But they are still missed in action. Sounds over the top. And makes everything only more overcomplexe in this language. Whose idea was that anyway?
