When the Gravemind sends Master Chief and Thel to find the Index he teleports them to High Charity and near the Activation Tower. How did he do that, is it because he had Penitent Tangent and was able to hack into the Installation’s teleportation grid? Also, if it was Installation 05’s tele grid, how did he get Chief onto High Charity?

You probably just explained it. Also Installation 05’s teleportation grid may have had a sphere type thing I.E. it could be that High Charity was in the right place to allow teleporting to it. That or he found a way to hack into their teleportation grid which is equally scary in its own right.
He had control over Penitent Tangent, so he probably also had a pretty good grasp on the Installation’s translocation grid.

This. Despite being an organic being, the Gravemind can get into computer systems. Look at how it was able to mess with Cortana’s systems in Human Weakness.
He used the translocation system on Installation 05, similar to what Cortana accessed on Installation 04 and Mantle’s Approach.

It’s also possible he hacked into high charity’s transport grid in order to place chief there either way it was just a plot point to get them to somewhere else fast.

I don’t believe High Charity itself has a translocation grid, but Installation 05’s likely stretched out to High Charity’s position.

Even if it didn’t naturally, there’s nothing that says the Gravemind couldn’t manipulate the systems to do that.
