How do you get your custom challenges to start registering in the game?
Do I have to sign out and sign back in for them to start working?
Custom challenges do not show up in game, only on the website. The kills/medals ect that you earn in them will show up on the site a few seconds after the end of the game/level you earned them in.

Darn, I was hoping I would be able to track my progress while in games.
Nope. You can’t view your progress in game. But if you keep your computer handy while playing on your xbox, between games you can refresh your page on halo waypoint.(remember to stay logged in after making the custom challenges) While in a game the website wont track the progress. But between games you can check to see how far left you have to go.
