Hi everyone. I was just curios if anyone else was having trouble with this achievement. I decided to finally come back and 100% this game but no matter what I do this achievement will not pop. I completed it three times on solo Legendary and once in co-op and it just will not unlock. On my most recent run, not a single member of my fire team died and I even did the Warden Eternal fight at the end without skipping it, but I still can not get the achievement. Does anyone else know what’s up with this?
Have you reverted to the last checkpoint when you died? That still voids the achievement. You have to quite, then reload.
I could have sworn that I read somewhere that you could revert to the last checkpoint. I am pretty certain I did not die at any point, but I did revert when I was downed to make sure that it did not count as a death if my fire team died. When you say that I have to quit and reload, can I reload to that checkpoint or do I have to restart the whole level?
I got it way back when… everytime my fireteam died and I was bleeding out, I would revert before I actually died… as long as you don’t force respawn to the previous checkpoint you should be fine
Maybe the cheevo bugged within one of the 37 updates they’ve applied

I just finished playing The Breaking in legendary difficulty and faced the same problem. Me and my friend played it in co-op and we both wanted to unlock the achievement. After the mission was over, the achievement was given only to him and not me. How is that possible ?
