I’ve completed the requisites for it twice now; get through the entire mission without the entire fireteam dying, once solo and once co-op and have not gotten the achievement. Has anyone else had a similar problem?

Did you went back to last checkpoint? The first time I tried it I didn’t get it because I restarted from last checkpoint.
