While playing a game of firefight, we lost due to the inability of any of using being able to actually obtain anything from the REQ machines. We could not get any guns, powerups, vehicles, etc. What caused that?
It is a current problem with 343 Industries servers that is causing problems with many players in different ways.

Do check that the problem is not on your end as well though. Perform a cold reset of your Xbox One by holding down the power button for 10 seconds to clear the game cache. Additionally, resetting Alternate MAC Address in system network settings can make the game a lot more responsive.
343 are aware of the current issues and are investigating.
