Is it dead? I haven’t received a survey in months. It may have been last year.
I don’t think it’s been shut down, as Grim said that there’d be an increase in surveys soon.
However, the feedback program does seem a bit quiet lately. I wish there was a way to check if you was confirmed as a member of the program. 
I got one a while ago. Maybe about two months ago…
