He didn’t Cortana updated it herself using Nanotechnology while John was sleeping in Cryo. She needed something to do to try and ward off her rampancy.
It’s not Mark IV, it’s Mark VI remodeled to look like Mark IV using the Nanotechnology mentioned above.
