I have seen this issue span from the date of release until now, i have completed all of the spartan ops missions solo legendary to find the achievements “Icarus” “Smooth operator” “feet first” “term of enlistment” “hazard pay” and “key” are still all locked. I then played through all of spartan ops on legendary solo AGAIN to find none of these achievements have been unlocked. Is someone able to help me please, I’m extremely frustrated at this point and halo is the only game I would spend my time grinding for achievements on and to find hours of my time have been for nothing is just defeating

An update to Spartan Ops is on the horizon. There have been several hints.
Just make a support ticket on Halo Support for now and quit while you are ahead. A broken game is a broken game. Nothing no one on this forum can do about it.
It’s likely you are one of the few who have encountered this issue because nobody plays Spartan Ops lol. It’s a multiplayer experience that has been castrated on the MCC.
To this day, it’s riddled with bugs that can prevent level progression let alone achievement progression.
And no matchmaking options means it is essentially a redundant game mode.
I had a similar issue. One chapter of one episode (Episode 5 Chapter 3) had glitched and was not being recognised as completed. Support chat and solution copied below for you.

Thank you for reporting these issues on your Halo 4 Spartan Ops Achievements for Episode 5. You can try these troubleshooting steps that may improve the issue for Achievements.
Please refer to this support article, for troubleshooting and more information regarding achievement issues. With that said, please confirm if there are any changes after doing the following:
Disconnect from the internet (go Offline) after launching Halo: MCC
Replay the mission/complete the achievement’s requirements
Complete the mission and return to the menus
Exit the game completely, Reconnect to the internet, relaunch the game and check if the achievement would now pop-up
Many thanks for your help. I tried this with chapter 5, then 4, then 3. After chapter 3 it instantly popped upon relaunching the game so I think that chapter had been the problem.
What updates are coming to the mode? The only thing that has happened to Spartan Ops in the past year in general was the mode got moved from the Campaign Menu to the Firefight menu. It seems for a long time now that the Matchmaking mode that was missing from the original might finally be making its return if Spartan Ops is in the Firefight menu now while the other two firefight games for Reach and ODST have this already, but there has been nothing regarding any kind of update about that from 343 at all.
That’s why I called it a hint. Sadly, it is still not confirmed.
What we do know for sure though is that Spartan Ops is getting Theater support. So, that definitely is a new update on the horizon for the mode.
Of course… this is granted that the skeleton crew who’ve been working on the MCC these past few years have not finally decomposed lol.
