I think survivor mod somehow caused me to lose the perfection im not sure all I know is I hope to get it back

There is no Perfection medal in Halo 4. Perfections count towards your Perfection commendation.

You are currently at Perfection - Proficient 1/2, which means you’ve gotten 2 Perfections.

Must have been the betrayal, those who splatter their own teammates are not worthy of the perfection.

And the good thing about it not being a medal, you get the commendation even if it blackscreens.

they wont give u a perfection with a betrayal because the guy u betrayed u killed his chances of gettin his perfection

I’m pretty sure you can get a Perfection commendation even if you accidentally betray. You just need 15 kills and no deaths. Betrayals won’t void it.
