So I beat the game on Legendary. Under the campaign lobby … missions… it says that everything is done. Even my achievements says I got the “Lone Wolf Legend”. However, if I go to Spartan Hub…career … commendations…player…campaign…I find that 3 of my levels are not completed… Did anyone has the same problem? Is that a glitch? What are my options? Can an administrator or 343 help me?

Completed Campaign but missing achievements
This issue was fixed in the 12.06 Title Update. Please download the necessary update to fully resolve the issue.
I found this on an official 343 board. I updated the games before playing, but it seems to not have worked for me.
Have you been connected to Xbox Live all the time? Your Halo 4 Sevice record says me that you haven’t completed Mission 01, 06 an 07.
The commendations can only be completed when you are connected to Xbox Live. And when you are missing a few commendations, then this is probably the reason for it.
Your only option is to play those missions again while connected to xbl.
if you lost connection while playing during those missions then you won’t get credit for passing it.

If you did lose connection while playing then you will have to redo them. I had to redo them myself for the mission midnight and shutdown. Im sorry bro. If you need help I can try and help Co-Op, but im not really good at campaign.

Yes to redo them. but you have to complete the campaign solo legendary to get the Raider DSTT Torso
